Resectoscopic Application in Gynecology





Abstract
The resectoscope has been used in urologic procedures for over three decades, but has been applied to gynecologic procedures only in the past decade. The purpose of resectoscopy is to reconstruct the uterine cavity and to produce hemostasis of the uterine cavity through the cervix. it has a wide range of uses, including resection of submusous fibroids, polyps, and uterine septae, and, more recently, ablation of the endometrial cavity.
From August 1990 to February 1992, 67 women with intrauterine disease underwent resectoscopic surgery under general anesthesia at l onsei Medical Center. Among the total of 67 patients, 7 underwent endometrial-ablation for intractable uterine bleedings and showed a 100% cure rate. Also, in 21 patients ablation along with myomectomy and/or polypectomy was performed, of whom 19 patients representing 90% showed complete cure.; Among the 14 patients who only underwent myomectomy and/or polypectomy without ablation the complete cure occurred in 10 patients representing 71 %. In 16 infertility patients pregnancy after myomectomy and/or polypectomy occurred in 6 patients representing 37%, of whom 2 patients delivered term babies, but in 9 patients with uterine septum, pregnancy occurred in only one patient(11 % ). There were no complications such as, bleeding, infection and fluid overload.
Therefore resectoscopy may be considered as a safe, simple and effective method of treating of intrauterine diseases.
